Formulir Baru Pernikahan Diberlakukan

Moh, Junaidu : Tentang penggunaan formulir Model N (Akta Nikah) telah disederhanakan.

LUMAJANG, EXTREMMEPOINT.COM : - Penggunaan formulir N dan NB yang baru untuk persyaratan pernikahan telah diberlakukan sejak 17 /01/2012 di Kabupaten Lumajang.

Kantor Kemenag Kabupaten Lumajang pada yang lalu telah melaksanakan Sosialisai Pembinaan Penggunaan Model N dan NB, hal tersebut menindaklanjuti atas surat Nomor DJ.II.2/1/KS.01.3/27/12/2011 29/9/2011 dari Direktur Bimbingan Masyarakat Islam Kemenag RI yang bertempat di Aula Kantor Kemenag Lumajang dan dihadiri oleh seluruh Kepala KUA dan Penghulu-penghulu se-Kabupaten Lumajang sekitar 50 orang.

Drs. Moh. Junaidu, MA, Kasi Urais menyampaikan, “Bahwa dalam rangka meningkatkan kualitas pelayanan pada masyarakat khususnya di bidang Nikah dan Rujuk di KUA Kecamatan, tentang penggunaan formulir Model N (Akta Nikah) telah disederhanakan, untuk itu dalam pelaksanaan pelayanan pencatatan NR supaya menggunakan formulir model N yang baru,” kepada extremmepoint.com.

Lembar ke 4 dari Formulir Daftar Pemeriksaan Nikah (model NB) yang lama diganti dengan Model N yang baru pada setiap peristiwa. Kepala KUA atau Penghulu supaya membawa model N untuk diisi dan ditandatangani para pihak.

Agar tertib dalam beradministrasi, Model N yang baru supaya dijilit dengan rapi kemudian disimpan sebagai dokumen Negara, sesuai dengan peraturan perundang-undangan yang berlaku, penggunaan model N dan NB yang baru terhitung mulai 17/01, dan berkaitan dengan perubahan penggunaan model N dan NB supaya membuat berita acara tentang sisa stok model N dan NB yang lama.

“Kepada Kepala KUA diharapkan, yang belum membentuk kepengurusan BP4 supaya segera dibentuk, akhirnya beliau mennnyampaikan marilah kita bekerja sesuai dengan peraturan yang ada supaya tidak menimbulkan permasalahan dikemudian hari,” pungkas Junaidu. (CKO)
